Assess, teach, and deliver therapy interventions directed by the physician while collaborating with interdisciplinary team members to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels to establish goals and outcomes. Provide written evaluations, modify treatment plans as needed, and coordinate discharge care including home exercise plans for patients and caretakers. Maintain accurate documentation to ensure compliance with physician orders and payer standards, while supervising PTAs, HHAs, and OTAs where applicable. Complete OASIS assessments and serve as a clinical resource consultant for staff. Work Monday through Friday full-time in the Wabasha, Winona, Buffalo, and surrounding communities.
