Home Health Physical Therapist-Per Diem

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and patient team goals. Provide clear written care instructions to PTAs and Home Health Aides, oversee their performance and documentation, and report patient progress to the physician and care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. This per diem role with CenterWell Home Health supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
