Job Summary
Deliver skilled physical therapy services to patients in their homes, developing and implementing individualized care plans in collaboration with the interdisciplinary team. Educate patients and caregivers on techniques and home exercises, complete evaluations, progress notes, and discharge summaries, and support care transitions and discharge planning. Requires Washington State licensure to practice, a valid driver’s license, CPR certification, and strong communication, critical thinking, and time management skills. Home visits in the Tri-Cities and Walla Walla areas with a Monday–Friday schedule and no required nights or weekends.
Required Qualifications
- Licensed to practice as a Physical Therapist in Washington State
- One to two years of experience in physical therapy; home health experience preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
