Job Summary
Physical Therapist in Homecare Therapy delivering 1:1 rehabilitation in patients’ homes within the Greater Nashua area. Evaluate functional needs, develop individualized treatment plans, restore function, reduce pain, and promote independence. Provide home exercise programs, document in the EHR, collaborate with physicians and Homecare Team, supervise PTAs, participate in interdisciplinary care conferences, and travel within the agency’s territory to visit patients. Requires NH licensure, CPR, and two+ years of hospital/clinic experience; advanced degrees (MS/PhD) preferred. The role emphasizes autonomy, teamwork, patient-centered care, and meaningful patient relationships. The position is full-time with Monday–Friday schedules and rotation for weekend/holiday coverage.
Required Qualifications
- Licensed Physical Therapist in NH
- Bachelor’s degree in Physical Therapy (minimum)
- CPR-certified
- two or more years of hospital or clinic experience
