Job Summary
Assess patients' functional abilities and home environments, develop individualized treatment plans, and guide targeted interventions to improve mobility, balance, strength, and safety. Provide one-on-one rehabilitation care in patients' homes, assisted living facilities, or nursing homes to help individuals recover from surgery, injury, or illness while reducing pain and supporting greater independence. Work closely with an interdisciplinary care team to educate patients and caregivers, ensuring measurable progress toward their goals. This high travel position requires daily travel within the Jackson, MI service area and is ideal for a licensed Physical Therapist with at least one year of experience who values autonomy and clinical variety.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
