Job Summary
Home Health Physical Therapist position in Jackson, MI, providing one-on-one rehabilitation care in patients’ homes to improve mobility, balance, strength, and safety; responsible for assessing functional abilities, developing individualized treatment plans, guiding targeted interventions, collaborating with an interdisciplinary care team, and educating patients and caregivers while maintaining autonomy and high-quality care.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area

- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
