Conduct home health physical therapy services in the Northern part of Colorado, providing 1-2 days of coverage per week in a mobile outpatient setting. Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies to help them regain mobility, independence, and overall wellness. This role features Part B–only documentation with less charting, allowing more time with patients.
