Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies to achieve optimal outcomes in a mobile outpatient setting. Provide high-quality therapy services to patients in their homes in the Northern part of Colorado, focusing on regaining mobility, independence, and overall wellness. Work directly with patients for 1-2 days a week with Part B–only documentation, allowing for less charting and more time with patients.
