Conduct home health physical therapy services for patients in the Northern part of Colorado, focusing on 1-2 days per week in a mobile outpatient setting. Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies to help them regain mobility, independence, and overall wellness. The role features Part B–only documentation with less charting to maximize time with patients. Weekly pay and schedule flexibility are included, along with a referral program for additional income.
