Assess patients' physical conditions and develop personalized treatment plans to help them regain mobility, independence, and overall wellness. Implement evidence-based therapies in a mobile outpatient setting with Part B–only documentation, allowing for less charting and more direct patient interaction. Provide high-quality therapy services to patients in their homes in the Northern part of Colorado on a 1-2 day commitment per week.
