Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies to achieve optimal outcomes in a mobile outpatient setting. Provide high-quality therapy services to patients in their homes in the Northern part of Colorado, focusing on regaining mobility, independence, and overall wellness. This role features Part B–only documentation with less charting and more time with patients. Commitment is 1-2 days a week.
