Home Health Physical Therapist - Now Hiring!

Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies in a mobile outpatient setting to empower patients to regain mobility, independence, and overall wellness. Provide high-quality therapy services to patients in their homes in the Northern part of Colorado for a 1-2 day commitment per week. This role features Part B–only documentation with less charting, allowing more time with patients.
