Job Summary
Assess patients' physical conditions and develop personalized treatment plans to help them regain mobility and independence. Implement evidence-based therapies in a mobile outpatient setting with Part B–only documentation, focusing on less charting and more time with patients. Work directly with home health patients in the Northern part of Colorado for 1-2 days a week to achieve optimal outcomes.
