Home Health Physical Therapist - Now Hiring!

Assess patients' physical conditions, develop personalized treatment plans, and implement evidence-based therapies to achieve optimal outcomes in a mobile outpatient setting. Provide high-quality therapy services directly with patients in their homes to empower them to regain mobility, independence, and overall wellness. This role features Part B–only documentation with less charting, allowing more time with patients. Commitment is 1-2 days a week in the Northern part of Colorado.
