Conduct comprehensive physical therapy evaluations in patient homes and develop individualized treatment plans focused on functional mobility, safety, and independence. Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development while performing home-safety assessments. Manage a field-based caseload with strong organizational efficiency, coordinate care with interdisciplinary team members, and maintain accurate documentation. Support safe transitions from hospitals, SNFs, and outpatient settings through effective communication with physicians and caregivers. This full-time, Monday–Friday role offers a manageable travel radius within the Caldwell community.
