Job Summary
Home Health Physical Therapist focused on the geriatric population, full-time, serving Northeast Austin and surrounding areas. Responsibilities include ongoing clinical assessments (including OASIS where applicable), developing and evaluating a plan of care with patients and caregivers, documenting and updating clinical notes and plans of care, coordinating with physicians and other care team members, meeting continuing education requirements, instructing patients and families on transfer techniques and equipment use, providing patient, caregiver, and family counseling and education, applying infection control and standard precautions, accepting assignments aligned with education and competence, providing services as ordered and supervising PTAs and home health aides as appropriate, and planning/treating neuromuscular, neurological, and orthopedic disorders per medical referral. Requirements include a current PT license; Homecare Homebase EMR knowledge is a plus; home health or related healthcare experience preferred; strong clinical/assessment and communication skills; ability to work independently and manage time.
