Home Health Physical Therapist

Conduct ongoing clinical assessments, including OASIS assessments, to identify patient needs and determine appropriateness for home care. Develop and evaluate plans of care in partnership with patients and caregivers, while documenting clinical notes, phone orders, and revising plans based on assessment data. Provide neuromuscular, neurological, and orthopedic treatments based on medical referrals, oversee physical therapy assistants and aides, and instruct families on transfer techniques and equipment use. Apply infection control standards, communicate with physicians, and meet mandatory continuing education requirements. Manage a fast-paced, independent schedule with a preference for one year of home health experience and knowledge of Homecare Homebase software.
