Home Health Physical Therapist

Deliver skilled, patient-centered care in a home health setting by performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies while working independently within an assigned territory. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation for all patient interactions. This role requires an active Physical Therapist with a current license and at least one year of experience, including the ability to spend approximately 20% of the day driving to patient locations in the West Newton, PA area. Successful completion of a drug screen is required prior to employment.
