Job Summary
Deliver skilled, patient-centered care in a home health setting by performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies while thinking critically to solve problems in real time and ensure thorough, compliant EMR documentation. Work independently within an assigned territory, traveling to patient homes to provide care and collaborate with an interdisciplinary team. Requires one year of experience, a current PT license, and a valid driver's license for ~20% of the day spent driving.
Required Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
