Job Summary
Residential Home Health and Hospice is seeking a Home Health Physical Therapist to join a team-based environment providing management and delivery of patient care plans in Philadelphia, PA and surrounding areas. The role requires a PT license in the working state, a Bachelor’s or Master’s degree in Physical Therapy, and at least one year of healthcare experience. Candidates must have a valid driver’s license and be able to spend about 20% of the day driving to/from patient locations. Responsibilities include collaborating to deliver patient care plans, charting, and utilizing EMR software on tablets and smartphones. Required Qualifications
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state
