Job Summary
Home Health Physical Therapist role: provide management and delivery of patient care plans in a team-based home health/hospice environment. 40 hours per week in Lebanon, PA and surrounding areas, with patient-centered care and collaboration with clinical educators, preceptors, and supervisors. Requires a Physical Therapy degree (Bachelor’s or Master’s) and current state licensure, plus a driving component (~20% of time) to visit patients. Proficiency with EMR software and mobile devices (tablets/phones) is needed, along with strong time management to meet charting deadlines. This position emphasizes compassionate, self-motivated patient care, utilization of a variety of benefits and onboarding resources, and the ability to work within a supportive care team to advance patient outcomes.
Required Qualifications
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state in which you will be working
- Minimum of one year healthcare experience
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- Commitment to consistently meet critical deadlines for charting
- Demonstrated experience with tablets, mobile phones and EMR software
