Job Summary
Provide management and delivery of patient care plans in home health and hospice, collaborating within a team-based environment to support patients in the Northland area. The role requires a Licensed Physical Therapist with KS and MO licensure, a Bachelor’s or Master’s degree in Physical Therapy, and the ability to drive to patient locations. Strong comfort with tablets/mobile devices and EMR software, as well as a demonstrated ability to meet charting deadlines, is needed. This full-time clinical position emphasizes patient-focused care, care-plan delivery, and coordination with clinical educators, preceptors, and supervisors to support career mobility and growth.
Required Qualifications
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
- Licensed Physical Therapist issued by the state(s) in which you will be working
- Must be licensed in both KS & MO
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- Demonstrated experience with tablets, mobile phones and EMR software
