Job Summary
Deliver skilled, patient-centered care in a home health setting by performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate patients and caregivers on therapeutic exercises and injury prevention while working independently within an assigned territory, traveling to patient homes. Think critically to solve problems in real time and ensure thorough, compliant EMR documentation. Requires an active Physical Therapist license, one year of experience, and a current driver's license. Includes a $20,000 total bonus potential with a $10,000 first-year success bonus and a $10,000 sign-on bonus.
Required Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
