Job Summary
Provide management and delivery of patient care plans in a home health setting as part of a collaborative Mary Free Bed care team. Collaborate with Mary Free Bed Rehabilitation Hospital in a team-based environment to deliver PT services across Stanton, MI and surrounding areas. Required to hold a licensed Physical Therapist credential in the work state and possess either a Bachelor's or a Master’s degree in Physical Therapy. Expect ~20% daily travel to patient locations, proficiency with EMR software and mobile devices, and a focus on timely charting and patient outcomes. Required Qualifications
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state in which you will be working
- Minimum of one year healthcare experience
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- Commitment to meet deadlines for charting
- Experience with tablets, mobile phones and EMR software
