Job Summary
Conduct comprehensive patient evaluations, including history reviews and specific assessments, to create tailored treatment plans for individuals recovering from surgery, injury, or chronic conditions. Implement treatment protocols and adapt techniques based on patient presentations while communicating findings and goals to patients and families. Collaborate with multidisciplinary healthcare professionals to optimize outcomes, manage discharge planning, and document all care activities for continuity. Maintain punctuality for home visits and internal meetings while adhering to regulatory standards and contributing to community engagement initiatives.
Required Qualifications
- A Doctorate in Physical Therapy (DPT)
- A valid Physical Therapist license in the state of Virginia
- Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
Desired Qualifications
- At least two years of relevant experience
- Experience within home health care or outpatient settings
