Job Summary
Conduct thorough evaluations to assess patients' physical capabilities and rehabilitation needs, including reviewing medical histories, performing functional assessments, and formulating comprehensive treatment plans. Implement evidence-based treatment protocols tailored to individuals' conditions and communicate effectively with patients and families about evaluation outcomes and treatment strategies. Maintain meticulous documentation of patient care activities, including initial assessments, progress notes, and discharge summaries, in compliance with healthcare regulations. Engage regularly in consultation with a network of healthcare professionals to adapt care plans based on patients' progress and evolving health conditions. This role focuses on adults and seniors requiring rehabilitation services following surgeries, injuries, or chronic conditions.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT)
- A valid Physical Therapy License in the state of Virginia
- Basic Life Support (BLS) certification from the American Heart Association
- State-specific licensing requirements as deemed necessary for practice
- Strong communication and interpersonal skills to connect effectively with patients and families
