Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function while administering tests to determine functional goals. Deliver physical therapy services complying with physician orders and maintain care plans through timely documentation, goal modification, and discharge planning with patient and caretaker education. Collaborate with the healthcare team, physicians, and nurses to coordinate services, supervise personnel as applicable, and complete OASIS assessments where permitted. Ensure all documentation adheres to policy and payer standards to prevent denials. Full-time Monday through Friday schedule in Warren, Shenandoah, Clark, and Fredrick communities.
