Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Provide written evaluations and plans of care, documenting client performance and modifying treatment as needed. Collaborate with physicians, nurses, and families to coordinate services and create discharge care plans. Supervise personnel such as PTAs and HHAs where applicable, and complete OASIS assessments per state regulations. Maintain effective communication across the healthcare team and family members while ensuring documentation meets payer standards to avoid denials.
