Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes. Maintain plan of care through timely documentation, modifying goals and treatment as needed, and determining discharge care plans. Collaborate with physicians, nurses, and caretakers to educate patients on home exercise plans and coordinate services within the Warren, Shenandoah, Clark, Warren, and Fredrick communities. Supervise personnel including PTAs and HHAs where applicable.
