Home Health Physical Therapist Full Time

Assess patient mobility, coordination, and strength to establish functional goals, then deliver physical therapy interventions directed by the physician's plan of care. Provide written evaluations defining needs, progress, and outcomes while documenting client performance and modifying treatment as needed. Collaborate with interdisciplinary team members, nurses, and families to optimize patient well-being and coordinate discharge planning with home exercise instructions. Complete OASIS assessments where permitted and supervise personnel such as PTAs and HHAs. Maintain effective communication with staff and healthcare providers while adhering to all licensure regulations and payer documentation standards.
