Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Administer, evaluate, and interpret tests to determine functional goals, then provide written evaluations and plans of care defining needs, progress, and expected outcomes. Deliver physical therapy services per physician orders while maintaining plans through timely documentation, goal modification, and discharge care planning. Collaborate with physicians, nurses, and family members; supervise PTAs and HHAs as applicable. Complete OASIS assessments where permitted and ensure documentation meets payer standards to prevent denials.
