Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services while complying with prescribed physician orders and maintain comprehensive documentation to prevent clinical denials. Provide written evaluations, modify treatment plans, and collaborate with physicians, nurses, and families to create discharge plans and educate caretakers on home exercise regimens. Supervise physical therapy assistants and home health aides where applicable. Complete OASIS assessments per state regulations and participate in care coordination across the healthcare team.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Drivers License
- Proof of Automobile insurance
- Required vaccinations
- Testing

- Experience in Home Health Therapy
- completion of OASIS documentation
