Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services compliant with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals. Provide written evaluations defining needs, goals, and expected outcomes, then maintain plans through timely documentation and modifications. Collaborate with physicians, nurses, and families to coordinate services, supervise personnel, and complete OASIS assessments. Execute discharge planning by educating caretakers on home exercise plans. Work full-time Monday through Friday in Warren, Shenandoah, Clark, and Fredrick communities.
