Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining therapy needs and expected results. Deliver physical therapy services per physician orders while maintaining plans of care through timely documentation and goal modification. Collaborate with interdisciplinary teams, physicians, and families to optimize patient well-being, manage discharge planning, and educate caretakers on home exercise plans. Supervise personnel such as PTAs and HHAs as applicable and complete OASIS assessments where permitted. Ensure all documentation meets payer standards to prevent denials and adheres to licensure regulations and patient care standards.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR certification
- Valid and current drivers lisence and proof of automobile insurance
- Full-Time visits needed weekly for Sioux City, IA communities
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
