Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Administer tests to determine goals and outcomes, then provide written evaluations defining therapy needs and expected results. Deliver physical therapy services per physician orders while maintaining plans through timely documentation and timely discharge planning with patient and caretaker education. Coordinate services with physicians, nurses, and family, supervise PTAs and HHAs as applicable, and complete OASIS assessments where permitted. Ensure all documentation adheres to policy and payer standards to prevent denials. Full-time Monday through Friday coverage in Hernando and Citrus Counties.
