Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy interventions directed by physicians within the home health plan of care while collaborating with interdisciplinary team members. Provide written evaluations, modify treatment plans, and manage discharge care including patient education on home exercise programs. Complete OASIS assessments, ensure accurate documentation to prevent denials, and supervise personnel such as PTAs and HHAs. Perform visits Monday through Friday across Virginia territories including Eveleth, Gilbert, and Mt. Iron.
