Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes, modifying treatment as needed to maintain discharge care plans. Collaborate with the healthcare team, physicians, and caregivers to coordinate services and educate patients on home exercise plans. Complete OASIS assessments where permitted by state regulations and document client performance to ensure no technical or clinical denials. Supervise personnel such as PTAs and HHAs as applicable.
