Home Health Physical Therapist Full Time

Conduct initial and ongoing mobility assessments to evaluate strength, coordination, and functional levels. Administer tests to determine goals, deliver therapy services per physician orders, and modify treatment plans as needed. Provide written evaluations defining needs and outcomes, while maintaining accurate OASIS documentation to prevent denials. Collaborate with physicians, nurses, and families on discharge planning and home exercise education. Supervise PTAs and HHAs as applicable, ensuring all policies and licensure regulations are followed.
