Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, modify treatment as needed, and create discharge care plans with patient and caretaker education. Perform timely documentation, complete OASIS assessments, and coordinate services with physicians, nurses, and the broader healthcare team. Supervise personnel including PTAs and HHAs where applicable. Maintain effective communication with staff and families while adhering to all licensure regulations and patient care standards. Full-time Monday through Friday schedule available in Warren, Shenandoah, Clark, and Fredrick communities.
