Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Administer, evaluate, and interpret tests to determine functional goals, then provide written evaluations and plans of care defining needs, progress, and expected outcomes. Deliver physical therapy services complying with physician orders while maintaining plans through timely documentation, goal modification, and discharge care planning. Collaborate with physicians, nurses, and caretakers to coordinate services and educate patients on home exercise plans. Supervise personnel such as PTAs and HHAs where applicable, complete OASIS assessments, and ensure all documentation meets payer standards to avoid denials. Full-time Monday through Friday schedule in Warren, Shenandoah, Clark, and Fredrick communities.
