Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function, then administer, evaluate, and interpret tests to determine functional goals. Deliver physical therapy services complying with physician orders while maintaining care plans through timely documentation, goal modification, and discharge planning with patient and caretaker education. Collaborate with physicians, nurses, and other healthcare team members to coordinate services, supervise personnel as applicable, and complete OASIS assessments where permitted. Ensure all documentation adheres to policy and payer standards to prevent denials, while maintaining effective communication with staff and families. Work full-time Monday through Friday in Warren, Shenandoah, Clark, and Fredrick communities.
