Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals. Provide written evaluations and plans of care, modify treatment as needed, and manage discharge planning by educating patients and caretakers on home exercise plans. Collaborate with the healthcare team, physicians, and nurses to coordinate services and document client performance per policy. Supervise personnel such as PTAs and HHAs where applicable, and complete OASIS assessments where permitted by regulation. Work full-time Monday through Friday in Warren, Shenandoah, Clark, Warren, and Fredrick communities.
