Home Health Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services compliant with physician orders while administering, evaluating, and interpreting tests to determine functional goals. Provide written evaluations and plans of care, modifying treatment as needed and determining discharge care plans. Collaborate with physicians, nurses, and caregivers to coordinate services and educate patients on home exercise plans. Supervise personnel such as PTAs and HHAs where applicable, and complete OASIS assessments per state regulations. Maintain effective communication with the healthcare team and family members to ensure accurate documentation and zero clinical denials. Full-time Monday through Friday schedule in Warren, Shenandoah, Clark, and Fredrick communities.
