Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while delivering physical therapy services in compliance with physician orders. Administer, evaluate, and interpret tests to establish functional goals, providing written plans of care that define needs, progress, and expected outcomes. Maintain discharge care plans by collaborating with healthcare teams, educating caretakers on home exercise programs, and supervising personnel as applicable. Complete OASIS assessments where permitted and ensure documentation meets payer standards to avoid clinical denials. Work Monday through Friday in Dekalb County and the I-20 Corridor, supporting a mission to revolutionize homecare delivery through personal, compassionate outcomes.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
