Home Health Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function while administering tests to determine functional goals. Deliver physical therapy services per physician orders, providing written evaluations and plans of care that define needs, goals, progress, and expected outcomes. Maintain discharge plans by collaborating with the healthcare team, educating caretakers on home exercise plans, and supervising PTAs and HHAs as applicable. Complete OASIS assessments where permitted and document client performance to ensure compliance with payer standards and minimize denials. Full-time Monday through Friday schedule in Warren, Shenandoah, Clark, Fredrick, and surrounding communities. Requires a Physical Therapy degree, current unrestricted state license, and valid CPR certification.
