Assess, teach, and deliver physical therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and modify treatment plans as needed, ensuring timely documentation to prevent denials and maintain compliance with physician orders. Collaborate with interdisciplinary team members, nurses, and families to optimize patient well-being, manage discharge planning, and supervise support staff where applicable. Complete OASIS assessments and maintain effective communication across all care stakeholders.
