Home Health Physical Therapist Full Time 10k Sign On Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Provide written evaluations and plans of care, modifying treatment and discharge plans as needed. Maintain documentation to ensure compliance with payers and policies, including OASIS assessments where applicable. Collaborate with physicians, nurses, and families on discharge education and home exercise plans. Supervise PTAs and HHAs as applicable. Perform Monday through Friday full-time visits in Sussex County with an $85–$113 hourly rate.
