Job Summary
Home Health Physical Therapist provides in-home therapy through PT assessments, treatment planning, and collaboration with an interdisciplinary team. Full-time visits Monday–Friday in IA (Des Moines area, with focus in Newton, Jasper, and Marshalltown). Requires PT degree and current state license; deliver and document therapy services per physician orders, develop/discharge plans of care, and coordinate with patients and families. Includes a $10,000 sign-on bonus.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
