Home Health Physical Therapist Full Time 10k Sign On Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Administer, evaluate, and interpret tests to determine functional goals, providing written evaluations and plans of care that define physical therapy needs and expected outcomes. Deliver services complying with physician orders while maintaining discharge plans through collaboration with physicians, nurses, and caretakers. Perform timely documentation and OASIS assessments to ensure compliance with payer policies and state regulations. Supervise personnel such as PTAs and HHAs as applicable. Full-time Monday through Friday visits in Kent County with a $10k sign-on bonus.
