Home Health Physical Therapist- full or part time available!

Evaluate patients and build individualized care plans, then adjust them as progress unfolds. Perform skilled therapy in patients' homes, keep physicians in the loop on progress and changes, and guide patients and families through home exercise programs. Supervise PTAs and HHAs as needed, stay current on documentation including OASIS and discharge summaries, and apply sound clinical judgment to every visit. Requires current CPR certification. Full-time or part-time availability offered with flexible scheduling.
